Anyone who wishes to write prescriptions for Clozaril must register with the CNR with the form entitled "Prescriber or Medical Director Registration Form" (download form via the
website, www.clozaril.com or call the CNR). The information required for registration includes name, facility name, address, phone number, DEA number, and a signature. The "Medical Director" of a facility may register his/her information by completing this form as well. This will allow any resident or
physician with privileges in a given institution (hospital, VAMC, state psychiatric facilities,
etc.) to prescribe Clozaril to an in-patient without individually registering him/herself.
Once a patient is discharged from that facility, the new prescriber is required to register the
patient under his/her DEA number as the current prescriber of record.
Once the form is completed, it can be mailed or faxed to the CNR.

B. Pharmacy
A pharmacist that wishes to dispense Clozaril must have his/her facility/store registered with the CNR, in order to purchase Clozaril from the wholesaler. When a patient presents a prescription for Clozaril, and the patient is new to the pharmacy, the pharmacist must verify that the patient is eligible and registered with the CNR, before dispensing Clozaril. For both new and existing patients, the prescriber must also supply the pharmacist with a current blood work result (drawn within 7 days irrespective of monitoring frequency) prior to dispensing.

C. Patient
The prescriber is responsible for registering any patient for whom s/he is writing prescriptions for Clozaril. The information required in order to register the patient in the CNR, and confirm the patient’s eligibility to receive Clozaril or clozapine, are: DEA and Name of prescriber, Initials of the patient, Social Security Number, Date of Birth and Gender.

a) by visiting www.clozarilregistry.com and requesting registration on-line OR
b) by calling the CNR at 1-800-448-5938
Once the prescriber and the patient are registered, the prescriber must obtain a WBC and ANC value for the patient. (Please refer to the easy to read chart to determine recommended increased monitoring and required discontinuance of Clozaril).
When a pharmacist receives a prescription for Clozaril for a new patient, the pharmacist must verify that the patient is eligible and registered with the CNR, before dispensing Clozaril. For both new and existing patients, the prescriber must supply the pharmacist with a current blood work result (drawn within 7 days irrespective of monitoring frequency) prior to dispensing.
